Deze is ontwikkeld als hulpmiddel, een handleiding, om effectieve paramedische interventies duurzaam te implementeren om dure zorg te voorkómen (preventie) of vervangen (substitutie). Deze routekaart is gebaseerd op de kennis over landelijke implementatie van EDOMAH en is in de EDOMAH praktijk uitgetest en geëvalueerd op haar mogelijkheid tot preventie en substitutie van dure zorg.
